The Clinical observation of 28 cases of Mucocutaneous Lymph Node Syndrome


Abstract
Author studied the clinical features and courses of 28 cases of mucocutaneous lymph node syndrome who were admitted to Seoul National University Hospital from 1975 to 1981. Among them, one case was transferred for cardiac consultation after the acute stage and the remainders were admitted at the acute stage. Following results were obtained.
1) Most of the cases were under 5 years and 46.4% were under 2 years. Male to female ratio was 1. 8 : 1. There was no significant difference in monthly incidence.
2) Mean duration of fever was 12 days with a range of 5-21 days. Skin rash was seen in all cases. The character was described as polymorphous erythematous maculopapular in 66.7%. Main involved sites were whole body and trunk. Cervical lymph nodes were palpated in
85.2%. The size of lymph node was mainly bean size. Other principal symptoms were conjunctival injection (96.3%), lip change (92-061), tongue change (88.9%) , oral mucosal change (81.5%), redness in extremities (77.8%) , indurative edema (96.3%) and desquamation
(81.5%) .
3) The other symptoms were cough, rhinorrhea, diarrhea, arthralgia, arthritis, vomiting, abdominal pain, aseptic meningitis and pericarditis.
4) In laboratory findings, anemia (34.6%), leukocytosis (53.8%) , increased ESR (87.0%) , positive CRP (90.5%) , negative ASO, increased a2 globulin (77.8%), proteinuria (20.8%) , pyuria (25.0%) , increased GOT (32.0%) , increased GPT (23. 1%) , hypoproteinemia (42. 9%) ,
hypoalbuminemia (71.4%) and stool occult blood (37.5%) were observed. Increased platelet count was observed in 2-3 weeks.
5) At the acute stage, auscultatory finding of the heart was tachycardia and no other abnormal findings were observed. EKG abnormalities except tachycardia were observed in 24.0%. The case for cardiac consultation had severe mitral regurgitation and normal coronary arteries.
